Introduction
Hi, my name is Bethany Hooker, and I am the school nurse for Shive. Please feel free to contact me if you have any concerns or any information that you feel I should be aware of to better serve your child and provide the very best care for them. My goal is to educate and care for your child so that together we can have a safe, healthy, and successful year.
PARENT REMINDER
***Please do not send your child to school if they are running fever or have been running fever in the past 24 hours. This includes giving them Tylenol or Motrin and sending them to school. They are still contagious as long as they are still running a fever.
***Please do not send your child to scool if they have had vomitting or diarrhea in the past 24 hours. Most likely it is from a virus, and they could still be contagious.
Medication
If your child needs to take medication while at school, please send medication in original packaging with a permission note stating child's name, date, and instructions. If it is a prescription medication, it must be in original packaging with the student's name on it. Thank you for your cooperation. I will not be able to give the medication if I do not have a permission note from a parent or guardian.
Annual Hearing and Vision Screenings for 5th Grade Student and Newly Enrolled Students
Each year 5th grade students and newly enrolled 4th grade students are screened for hearing and vision. If your child fails the first round of testing, I will rescreen them 3 weeks later. If they fail either test at that point, I then send home a referral for them to be seen by a doctor. I appreciate you having this seen about promptly if there should be a problem. I encourage the kids to take these test very seriously and to try their best because proper vision and hearing are essential for successful students.
TIME FOR FLU SHOTS!!
It's that time again and I greatly encourage all students and parents to get their flu shot. If you have any questions, feel free to call me, our local Department of Health and Human Services, or your family physician.
